Forms & Paperwork

Every form you need, in one place

Bail paperwork looks intimidating and is usually simpler than it seems. Here is what each form does, what to have ready, and how a licensed agent completes it with you — day or night.

How it works

Step 1

Call before you fill anything out

One conversation tells us which forms actually apply to your case. Most families need two, not eight.

Step 2

Sign from wherever you are

We send the forms by email or text for electronic signature, or meet you at the jail if you prefer paper.

Step 3

We post the bond

As soon as the paperwork is signed, a licensed agent posts at the facility and we track the release with you.

The forms, explained

Ask us to email or text any of these and we will send the fillable version, in English or Spanish.

  1. Start here01

    Bail Bond Application

The intake form behind every bond we write. It records the defendant, the booking facility, the charge, the bail amount, and who is arranging release.

Your agent can fill this out over the phone in a few minutes and send it back for an electronic signature, so nobody has to drive to an office in the middle of the night.

    Have ready

    • Defendant's full name and date of birth
    • Jail or booking facility
    • Bail amount or case number if known
    Request this form — Call (714) 515-5154
  2. Co-signer02

    Indemnitor Agreement

Signed by the person guaranteeing the bond. It spells out, in plain language, what a co-signer is responsible for while the case is open.

We review each line with you before you sign — including what happens if a court date is missed and how the agreement ends once the case is resolved.

    Have ready

    • Valid photo ID
    • Proof of residence or employment
    • Two personal references
    Request this form — Call (714) 515-5154
  3. Payments03

    Premium Payment Plan

Used whenever a family finances the premium instead of paying it in full. It sets the down payment, the schedule, the balance, and the accepted payment methods.

Plans are written around your budget, and qualified clients may be eligible for our discounted rates and 0% financing options.

    Have ready

    • Down payment amount
    • Preferred due dates
    • Card, transfer, or cash details
    Request this form — Call (714) 515-5154
  4. Collateral04

    Collateral Receipt & Release

Most of our bonds require no collateral at all. When it is used, this form documents exactly what was pledged and the conditions for returning it.

You receive a signed copy the same day, and the release is processed once the court exonerates the bond.

    Have ready

    • Description of the pledged item
    • Ownership documentation
    • Signature of the owner
    Request this form — Call (714) 515-5154
  5. Property bonds05

    Property Bond Worksheet

For larger bail amounts secured by real estate. It gathers the property description, recorded ownership, and current equity so the court can evaluate it.

Property bonds take longer than a surety bond, so call first — an agent will tell you whether it is actually the fastest route in your county.

    Have ready

    • Recorded deed
    • Recent mortgage statement
    • Property tax assessment
    Request this form — Call (714) 515-5154
  6. Authorization06

    Credit Card Authorization

Authorizes a one-time or scheduled charge for the premium. Required for any payment taken by phone.

Card details are collected directly by a licensed agent and never left on voicemail, email, or text.

    Have ready

    • Cardholder name and billing address
    • Authorized amount
    • Cardholder signature
    Request this form — Call (714) 515-5154
  7. Third party07

Power of Attorney / Authorization to Act

Lets a relative or friend handle paperwork on behalf of someone who cannot be present, such as a co-signer out of state.

Commonly paired with a notarized signature; we will tell you when notarization is genuinely required and when it is not.

    Have ready

    • ID for both parties
    • Written scope of authority
    • Notary if requested by the court
    Request this form — Call (714) 515-5154
  8. Ongoing case08

    Court Date & Contact Update

Keeps a defendant's address, phone number, and next court appearance current for the length of the bond.

Filing this the moment something changes is the single easiest way to keep a bond in good standing and avoid a forfeiture.

    Have ready

    • New address or phone
    • Upcoming court date
    • Case or bond number
    Request this form — Call (714) 515-5154

Common questions about the paperwork

Do I have to come to an office to sign?
No. Forms can be completed and signed electronically by phone, email, or text, and an agent can also meet you at the jail or courthouse.
Is collateral required?
On most bonds, no. When collateral is used, it is documented on a receipt and released after the court exonerates the bond.
Are the forms available in Spanish?
Yes — live agents fluent in English and Spanish are available 24 hours a day, 7 days a week, and will review every line with you.
Who is licensed to write the bond?
Future Bail Bonds is a licensed California bail bond agency, license #1K15058. Only a licensed agent can execute and negotiate bail.

This page is general California bail bond information and is not legal advice.
